We also collect and process your Personal Information, including information necessary for and related to lab tests ordered by your healthcare provider, including the results of your lab tests and alerts for follow up with health care provider.

Test order history;

demographic information, such as age, gender and mailing address;

information about your sexual orientation, your sexual history and other relevant behaviors, if you purchase particular Services and test results indicate you may have a sexually-transmitted disease;

information about your medical history gathered through medical questionnaires you may complete in connection with the Services;

information discussed about patients during confidential phone calls with our lab techs, if the phone call is among those that are recorded from time to time for quality control purposes;

payment information, which is processed by our PCI-certified payment processing vendor;

any other information you may choose to submit to us.

See the details of each lab test for more information on the specific provider that reviews the test. We may also share Personal Information with trusted third party service providers that help support our website or other operations, including to help perform statistical analysis or provide customer support. We restrict all third party recipients, including testing labs, from using Personal Information they collect or process on our behalf except to provide these services to Emeraldnational.com and in compliance with our privacy policy and applicable law. They are required to maintain the confidentiality of customer information. Separately, health care providers are bound by health privacy laws, such as HIPAA, to protect personal health information.

We may share your Personal information as required or permitted by law to comply with a subpoena or similar legal process or government request, or when we believe in good faith that disclosure is legally required or otherwise necessary to protect our rights and property or the rights, property or safety of others, including to advisers, law enforcement agencies, judicial and regulatory authorities.

We may also share your Personal Information with state and municipal health departments in the event your test results indicate you may have a disease or condition that we are required by law to report to them. Not all jurisdictions require such reporting. In the event we are required to disclose your Personal Information to a health department, we will advise you of that fact when we notify you about your test results.

We may also transfer your Personal Information to a third party in the event of a merger, reorganization of our entity or other restructuring.

Your Privacy Rights

Test results are available within 1 to 3 business days after your processing request. Once your patient’s results are ready, you will receive an email at the email you designate from “Emeraldnational.com” letting you know that lab test results are available to view and download in your online account. We will not leave you voicemails or send you text (SMS) messages unless you explicitly consent to be contacted in that manner. 

Security

We use various methods to ensure the protection of your Personal Information against accidental or unlawful destruction, loss or alteration and against unauthorized disclosure or access. This includes encryption with industry-standard 128-bit encryption to protect your information and transactions while on and using our Site, and the health industry’s standard HL-7 interface. This helps protect your information whether you call our counselors to complete your order or place your order online—we work to keep patient data safe and confidential throughout the entire process. 

Data Retention
